dry eyes and dlk

Postby Karen » Sat Jan 27, 2007 10:09 pm

I had Lasik surgery done just over a month ago. I had a terrible experience with the first part of the procedure when they were making the flap. No one told me how unbearably uncomfortable it could be. I really thought I wasn't going to be able to stand it. Afterwards, there is a 15 minute wait before the laser is done. While waiting, I started shivering and my teeth were chattering...I think I went into shock. Anyways, afterwards the actual laser part of the surgery wasn't too bad (in comparison to that first procedure!)
My prescription prior to the surgery was -2.25 in one eye and -3.5 in the other with -6 diopters of astigmatism in both eyes. The first 2 weeks after surgery I was seeing pretty well. Outside everything was crystal clear; however, inside things were hazy, faces were not clear. I wanted to know why that was.
About 2 weeks ago, I was started to notice that my vision in general was not as good as right after the surgery. (I had been going to my optometrist for weekly visits). I never really noticed the blurriness until I was reading the eyechart. Now I was noticing in day to day situations. My doctor sent me back to the surgeon. He told me my cornea looked like an orange peel. After seeing the surgeon again, (he's a corneal specialist), my eye doctor told me that I had really dry eyes and a mild case of DLK. We waited a week to see how things progressed. My last visit to the surgeon he told me he wasn't concerned, thought the inflammation should clear up and put me on steroid drops for 2 weeks. I've been on them over a week and have noticed no change in my eyesight. There are some days when it seems worse. He also gave me a prescription for glasses to help me through...I finally broke down and got them.
Not at all what I was expecting after the surgery. My prescription isn't that bad, my nearsightedness is under 1 in both eyes as is my astigmatism.
Are you familiar with this happening to others. Will my eyes continue to get worse? I guess I just have to patient....
Your thoughts would be appreciated.

The steroids are to keep the Diffuse Lamellar Keratitis (DLK) at bay and reduce inflammation. The dry eye would easily explain the degradation in vision and the "orange peel" look of your corneas. You will want to read our article on Lasik dry eye treatment.

What you describe are known issues that normally resolve with treatment and healing. You will need to be diligent about your medications and your dry eye treatment, but it is reasonable to expect improvement over the coming weeks.

Just to keep you updated...the steroid drops did help with the inflammation, so I no longer take them. However, my eyes are very dry. The doctor seems to think that the steriod eye drops further aggrevated that. I go back in 3 weeks to see if there is any improvement. I am now wearing glasses pretty well all the time. My vision in my right eye is 20/100 and in the left it is 20/80. Trying to remain positive and hope that as the dry eye clears up (it will, won't it?!) my vision will improve. A little discouraged though because the day after surgery my vision was 20/40. Time will tell...

Karen,
I hope your vision gets better.

Glenn,
I have several questions.
Since my right eye is still blurry.
I am wondering how do you determine DLK?

1. Does it hurt or just blurry vision?
2. I am 21 days after surgery, since my right eye still blurry, is there possibility that this might be DLK?
3. I went to Doc twice so far but he looked at my eyes and told me they are ok, but is there special device that detects DLK?

Thank you in advance.

Well, it's been quite some time since I wrote. My vision is pretty well 20/20 now. I'm really happy with that.
I have had a lot of problems with dry eye...had a plug put in my right eye and that seemed to help a bit. Now I'm going to get one in the other eye. Still wake up with them dry - but no longer stuck shut.
My question is regarding blurriness. Pretty well every morning my right is really blurry. Can't even read the computer. Sometime around noon I notice it just clears up. Any ideas? Some days it just drives me crazy. My eye doc just said that for some things there are just no answers and this is one of them.
What do you think? Any help/advice is greatly appreciated.

Hey there karen, glad to hear you are doing well. How long was it before you were seeing 20/20 again? I had lasik done on Feb 27 and though i was seeing 20/20 the day after, i have gotten worse and started to get better. The not being able to see peoples faces thing is really starting to get to me as well. Looking at nature is ok, but reading things and seeing people but not seeing their expressions is rough. Just curious how long it was until you could see well again. Did you have double vision or ghosting while you were healing.

Btw, have you tried taking anything like restasis for dry eye or using warm compresses to get your tears moving. Plus this is the allergy season, maybe some of yours sympotms are allergy related. Hope you continue to improve.

Hey Griffin,
I actually had the lasik procedure done in Dec of 06 and again in April of last year. It's been a long healing process for me as I had really bad astigmatism to begin with and then some complications afterwards.
It was probably during the summer that I started seeing about 20/30 and then my vision starting getting worse..again - but found out it was due to damage caused from the dryness. That's why I got the plug put in. - and then the vision started improving again.
I'm just really frustrated right now with the morning blurriness...and there seems to be no cause for it....something i may just have to live with.
It hasn't been very long since you've had it done...be patient...I remember the blurry faces too, mine took longer than normal to heal because I had dlk and other issues.
I haven't tried what you mentioned...i will look into it. My eyes have been dry ever since I got it done...I don't think allergies are a problem. Thanks for the note.
Good luck to you.

Hi Glenn,
I had a question about DLK. I read about it on this web site. Is this something that can happen months/years after Lasik just out of the blue? Or does it only occur after there has been some trauma/manipulation of the flap.....months/years after? Is this something we should always have in the back of our minds as Lasik patients? Thanks.

Karen....thanks for sharing your story. Good luck with your morning bluriness. Sounds like you had a long healing process. Are you overall happy with the results?

Overall I am happy with the results. I mean, I couldn't see the big E on the eye chart prior to the surgery.
I didn't mention that last April I had to have it done over again. It's been since then that my vision has improved. I guess the astigmatism was just so bad.
The second time thru it wasn't as bad as the first. I didn't have to have to flap cut again, they peel it up.
It's funny, somedays the bluriness is so bad and today...not so bad at all. Very frustrating.
